Children must be at least 2 years old before the first day of the session.
All Parents/Caregivers participating in the classroom must submit:
Negative TB Test or Physician’s Risk Assessment,
Immunization Record for Measles,
Immunization Record for Pertussis (DTAP),
Flu Vaccination or Waiver,
